Complete items 1 through 7,
filling in your complete name, sex, social security number, birth date, residence address,
preferred mailing address (if different from your residence, use either a street address
or a post office box/drawer), home telephone number, alternate daytime telephone number,
and FAX number (optional). |

Read and answer yes or no to
ALL questions 8 through 13. |

In question 14, indicate
whether or not you are an Arkansas resident. |
|
In question 15, indicate
whether you are applying for the state license, certified residential or certified general
examination. |
|
In question 17, indicate on
which date you wish to take your examination. |
|
Mark question 18 ONLY if you
are requesting a non-Saturday test administration for religious reasons or special test
administration accommodations because of disability. Include a letter explaining you
request. |

In question 19, indicate if
your would like your score report mailed by an overnight courier. This requires an
additional express score reporting fee. (See Fee Schedule) |

In question 20, your
examination fee of $100.00 has been pre-printed after A. Write in the
total amount of the fees and include a money order, certified check, or cashier's check
for this amount with your application, payable to the Professional Examination Service. If
you pay your fees by money order of cashier's check, your name MUST
appear on your money order or cashier's check. Personal and business checks WILL
NOT be accepted. |
REMEMBER
You must also include with your application materials a separate money order, business
check, or personal check payable to the Arkansas Appraiser Licensing and Certification
Board for the application fee of $125.00.

and then SIGN and date your application. Your application is not complete until you have
signed it. |
