The information contained within this database is provided in response to your request for claims history on individuals who have either filed a claim for workers' compensation benefits or have had an employer reported injury resulting in lost time from work in excess of seven (7) days. Although the commission strives to provide accurate information, there is no guarantee made or implied that the data is error free. Also, pursuant to state law, no information can be released pertaining to reports on, or claims by , workers who have contracted or are tested for Acquired Immune Deficiency Syndrome (AIDS) or the presence of the HIV (Virus). Additional and more complete information is available by writing the commission and requesting file copies. The requestor should also be aware that, even though a claim is filed or an employer's report submitted, Workers' Compensation Benefits to the worker may not have been paid.
