IMPORTANT NOTICE: Effective June 11, 2012, ALL applicants must apply for a criminal background check. This includes current and previously licensed applicants. The Board must have the results of the Criminal Background Check prior to review of the application. You will be sent the Criminal Background Check instructions once your application is received.

Licensed Social Worker (LSW) {includes Provisional}

Requirements: Has a baccalaureate degree (BSW) in a social work program accredited by the Council on Social Work Education or receives before June 17, 1986, a baccalaureate degree in a social work program from an accredited educational institution.

Licensed Master Social Worker (LMSW) {includes Provisional}

Requirements: Has a master's degree (MSW) from an accredited social work program in an accredited institution approved by the Council on Social Work Education.

Licensed Certified Social Worker (LCSW) {Provisional not available}

Requirements: Has a master's degree (MSW) from an accredited social work program in an accredited institution approved by the Council on Social Work Education; Has two (2) years (4,000) hours post-graduate LCSW supervised social work experience (clinical or non-clinical).
